The San Francisco Continuum of Care covers the City and County of San Francisco. Approximately one thousand persons representing non-profit homeless assistance providers, business organizations and local and federal government, as well as homeless and formerly homeless persons were involved in the development of the Continuum of Care guided by the Task Force, a 35 person advisory body. The key element of the Continuum of Care is the establishment of integrated, effective and coordinated system of health care, housing, employment, and supportive services to prevent and reduce homelessness in San Francisco. This includes plans to develop a supportive housing production program, establish an integrated system of training and employment services, provide accessible and integrated substance abuse and health care and provide centralized and up-to-date information on housing, health, employment and social services for homeless persons.
