The Metro Dade County, Florida, Continuum of Care covers the entire county including the following communities: Hialeah, Miami, Miami Beach and North Miami. The Dade County Homeless Trust, charged with the responsibility of developing the Continuum of Care, consists of a 27-member Advisory Board comprised of representatives from State and local governments, businesses, nonprofit providers, educational and religious leaders, and formerly homeless individuals. Key elements of the Continuum of Care are: (1) outreach, assessment and placement of homeless persons into primary care (transitional housing); (2) advanced care (permanent housing); (3) general supportive services, and (4) coordinated case management and information/referral system.
