City and County of Honolulu, HI

"Partners in Care", the Continuum of Care strategy for the City and County of Honolulu, HI, is the product of a long-term cooperative alliance that began in 1986 among public and private organizations and individuals throughout the state. More than 60 representatives of community agencies, foundations, businesses, and government agencies participated in the development of the initial Partners-in-Care strategy in 1996. This effort was refined in 1997 under the direction of a special advisory board that was appointed by the mayor of the City and County. The strategy emphasizes a client-centered approach to service delivery through intensive case management and a shift in focus from twelve-hour shelters to longer-term transitional, permanent and supportive housing.
