The Greater Toledo Continuum of Care covers the City of Toledo, Ohio. Over 50 service providers, housing developers, lending institutions, and governmental entities were involved in the development of the Continuum of Care which was guided by the Greater Toledo Steering Committee that has committed itself to performing the "hands-on" work necessary to develop a permanent body to represent the Continuum. Key elements of the Continuum of Care are: (1) outreach to and identification of homeless persons and homeless veterans suffering from mental illness and substance abuse; (2) develop a seamless system of supportive services for homeless persons, chronically homeless persons, and those at-risk of becoming homeless; and (3) develop and implement a management information system network (ANCHOR) which will allow the Continuum to produce an accurate profile of Toledo's homeless population and subpopulations.
