The Luzerne County, Pennsylvania area Continuum of Care covers Luzerne County, Wilkes-Barre and Hazleton. Over 50 homeless participants including two local homeless coalitions, local government, public and private not for profit organizations, financial institutions, business groups, housing and supportive service providers, and homeless as well as formerly homeless individuals were involved in the development of the Luzerne County Continuum of Care which was guided by the Luzerne County Office of Community Development. Key elements of the Continuum of Care are: (1) conduct outreach to expand awareness of existing supportive service resources, (2) provide permanent affordable housing opportunities for individuals and families, and (3) examine existing service provision to identify potential areas for expanding availability of programs and services through increased networking, alternative service delivery or service enhancement.
