The Chester County, Pennsylvania are Continuum of Care covers Chester County. Over 45 sate and local government agencies, public and private for-profit and not-for-profit homeless assistance service and housing providers, consumers, foundations, community groups, and volunteers were involved in the development of the Continuum of Care which was guided by the Office of Housing and Community Development(OHCD). Key elements of the Continuum of Care are: (1) to develop a partnership between OHCD and the service community to plan and implement the components of the County['s Continuum of Care; (2) develop a uniform monitoring system that allows for assessment of existing facilities, and (3) to provide tools needed for homeless and low-income individuals to participate in Chester County's overall economic prosperity.
