The City of Chattanooga Continuum of Care Plan for the Homeless is designed to serve the homeless in Chattanooga and Hamilton County. The Chattanooga Homeless Coalition consists of 51 members that provide some type of service to the homeless and facilitate access to services through networking. The vision is to develop a comprehensive and continuous service system which can sustain engagement, housing and supportive services and an appropriate level of self-sufficiency for each consumer and his/her immediate family. Case managers will be able to track consumers needs and self-sufficiency progress across agencies.
