Houston/Harris County, Texas

The Houston, Texas, Continuum of Care covers the Cities of Houston, Baytown, and Pasadena as well as Harris and Fort Bend Counties. Over 60 non-profit homelessness assistance providers, business representatives, foundations, homeless and formerly homeless individuals, and government officials were involved in the development of the Continuum of Care which was guided by the Coalition for the Homeless of Houston and Harris County. Key elements of the Continuum of Care are: (1) development of formal and informal multi-agency collaborations; (2) linking of information, processes and program participants among the collaborating and independent agencies; (3) evaluation of program effectiveness and participant outcomes; (4) encouragement of a variety of agency types to address the needs of similar sub-populations; and (5) identification of and access to multiple funding sources which can be integrated into a comprehensive funding system.
