The City and County of Milwaukee, Wisconsin

The Milwaukee Continuum of Care covers the City and County of Milwaukee. Over 42 persons, representing homeless people and 40 organizations including non-profit homeless assistance providers, business representatives, and government officials were involved in the development of the Continuum of Care which was guided by the Emergency Shelter Task Force through a consensus driven process sponsored by Milwaukee County. Key elements of the Continuum of Care are: (1) implementation of a Centralized Information System that improves data collection, identifies and integrates existing community resources and services and efficiently links those services to homeless people (2) better coordination of the existing system, enabling homeless persons to more readily move though it toward self-sufficiency, and (3) development of additional housing for specific target subpopulations and services in specific areas which support the goal of achieving greater self-sufficiency.
