The Dane County Continuum of Care covers the City of Madison and Dane County. Over 31 persons representing homeless people, non-profit homeless assistance providers, business representatives, and government officials were involved in the development of the Continuum of Care which was guided by the Homeless Services Consortium. Key elements of the Continuum of Care are: (1) decreasing the need for and number of emergency shelter beds for disabled persons, (2) increasing the number of affordable transitional and permanent housing beds, and (3) increasing the number of case managers who provide outreach and follow-up services.
