The Charleston, West Virginia area Continuum of Care covers the City of Charleston and the County of Kanawha. Over 35 homeless people, non-profit homeless assistance providers, business representatives, neighborhood groups, and government officials were involved in the development of the Kanawha Valley Collective Continuum of Care which was guided by the Community Council of Kanawha Valley and the State of West Virginia Office of Economic Opportunity. Key elements of the Continuum of Care are: (1) plans for developing a universal intake and assessment system, and linking participating agencies together through computer networks and the Internet, (2) develop a centralized, integrated case management system coordinated between agencies, and (3) create an outcome based system to evaluate effectiveness of services on an ongoing basis and (4) develop services geared towards establishing responsibility and independence.
