made Jack's story a success story

Case Manager, Richard Golik, started working with Jack when he was homeless and living on the streets of Des Moines. Jack had a serious issue with alcohol and his 250+ arrests for public intoxication were costing Polk County thousands of dollars to house him in jail. In July 2005, Richard helped Jack get his own apartment through the Shelter Plus Care Housing Program administered by Anawim. Jack was very excited and thankful to Primary Health Care for their help getting him off the streets, and has only been arrested one time since. Having permanent housing has made a positive impact on Jack. In addition to reconnecting with his family, Jack is very proud of his apartment - keeping it neat and clean. He works hard to earn money to pay for his utilities.

The little clinic that could actually does a lot. The Outreach Project of Primary Health Care, Inc. (PHC) has been providing healthcare and social services for special populations since 1988. Operating inside Greater Des Moines' most urban and underserved area, the Outreach Project provides medical, social services, and homeless advocacy to thousands of vulnerable individuals - the uninsured, minority, women and children and homeless persons. Last year alone, The Outreach Project served 4,850 individuals by providing 5,300 medical visits, 5,227 family case management contacts and 25,860 homeless support services contacts.

PHC receives a Shelter Plus Care grant from HUD of $89,000 annually. Richard Golick, a PHC Street Outreach Counselor, said: "This grant from HUD has been tremendously successful; PCH has been able to take chronically homeless off the streets and placed them in housing."
