City of Phoenix Housing Department --- $178,121

The City of Phoenix Housing Department will be funded for a One-year Service Coordinator Renewal Grant in the amount of $178,121. The Housing Department will provide services to 400 elderly and non-elderly disabled public housing residents in 4 apartment communities. This group of resident has a vast array of on-going needs that must be addressed. The service coordinator links these residents to the necessary support services to ensure that they can continure living independently. The service coordinator also assists residents with their independent living needs, i.e.: business or medical correspondence, finances, telephone service, emergency food boxes, counseling, transportation, and socialization.
