Miami-Dade Housing Authority --- $230,000

The Miami-Dade Housing Authority received a $230,000 Service Coordinator Grant. This Service Coordinator Program is providing coordination of social services for a large population of frail elderly and younger disabled residents living in some of our oldest public housing facilities. All the sites that have been chosen are located in the same geographic area with the largest concentration of elders in Dade County. A needs assessment was conducted by Florida International University, which clearly indicated that this particular population is at high risk for institutionalization due to lack of coordination for social services beyond just the facilitation of housing. Professional Social Workers are providing service coordination and case management to over 5000 residents in 44 elderly developments.  

Fort Myers Housing Authority --- $42,600

The Fort Myers Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $42,600.00. The Housing Authority will continue to provide service for the Service Coordinator Program for the elderly and/or persons with disabilities. These funds will be used for employment and support of service coordinators in public housing developments designated for the elderly and/or person with disabilities. These elderly and disabled service coordinators help residents obtain supportive services that are needed to enable independent living and aging in place. This grant will provide service for three elderly public housing developments totaling 319 units.
