St. Clair County Housing Authority --- $33,320

The St. Clair County Housing Authority located in Belleville, Illinois will be funded for a One-year Service Coordinator Renewal Grant in the amount of $33,320. The Housing Authority will contract for service with the Belleville area colleges program and service for older persons. These funds will enable elderly and/or disabled residents to live independently, participate in and contribute to the community and in their own resident housing development.

Housing Authority of Champaign County --- $25,680

The Housing Authority of Champaign County received a One-year Service Coordinator Renewal Grant in the amount of $25,680 The Service Coordinator provides supportive services for elderly and non-elderly disabled residents at the five-elderly/disabled public housing communities. The target population includes the severely mentally ill, substance abuse (alcohol and drugs), physically disabled, and elderly residents. Clients already linked with services/case workers at the Mental Health Center or other social service agencies will be referred to them as necessary. Activities and groups will be open and participation will be encouraged from all residents.  

Housing Authority of the County of Cook$225,000

The Housing Authority of the County of Cook will be funded for One-year Service Coordinator Renewal Grant in the amount of $225,000. The Housing Authority will employ four Service Coordinators to provide service to over 1,400 elderly and non-elderly disabled residents in suburban Cook County. The coordinator's responsibility will include networking with local, state and federal agencies as well as organizations in the private sector. The goal and objective of the Hosing Authority will be to ensure the elderly and non-elderly disabled residents have access to the services they require to continue to live and prosper independently. The program will emphasize socialization, health awareness and crisis intervention.  

Chicago Housing Authority --- $600,000

The Chicago Housing Authority (CHA) will be funded for a One-year Service Coordinator Renewal Grant in the amount of $600,000. The Housing Authority will provide services to living in Senior Housing Communities to improve their quality of life. This objective is ensure that seniors have access to services that are needed, provide activities in the buildings to decrease isolation and secure appropriate counseling services as needed for people dealing with grief, loss, alcoholism and other problems. On a daily basis, Resident Service Coordinators identify the needs of senior residents allowing them to continue to enjoy independent living.  

Decatur Housing Authority --- $37,426

The Decatur Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $37,426. The Housing Authority will continue to employ a full-time Service Coordinator for the elderly and disabled Public Housing residents. The Coordinator will arrange for services such as medication management, health services, transportation, personal care, housekeeping, chore assistance and services as appropriate that assist residents in enhancing quality of life and avoid premature or unnecessary institutionalization. Funding will also be used for the Coordinator's salary, benefits, allowing the Authority to continue a highly successful Service Coordinator Program.
