Housing Authority of East Baton Rouge Parish --- $24,873

The Housing Authority of East Baton Rouge Parish received a Service Coordinator Grant in the amount of $24,873. This Service Coordinator will be used to provide case Management and Referral Service to our Elderly/Disabled Residents, monitor on-going provisions of service from the community, establish and network with local service providers to expand services for the targeted population, organize and/or facilitate workshops to educate the clients on available services. To ensure a continuum of services, the Coordinator will also train and inform existing staff working with the elderly/disabled residents.
