New Bedford Housing Authority --- $43,497

The New Bedford Housing Authority's grant of $43,497.00 will be used for the continuation of the Service Coordinator Program for Elderly and Persons with Disabilities. The current program has proven to be very successful. The importance of this program has increased as the general population of elderly and disabled persons has increased. Although this population becomes steadily frailer and they are often at risk, these residents desire to maintain their quality of life. The continuation of this program will assure that these residents are linked to the supportive services that they need to continue living independently.

Lynn Housing Authority --- $44,880

The Lynn Housing Authority will be funded for a One-Year Service Coordinator Renewal Grant in the amount of $44,880. The Housing Authority will continue employing an Elderly Services Coordinators to maintain the success achieved in addressing the critical needs of this specific population. Working with the residents, the Coordinator will continue working with Lynn's provider community to provide leveraging of appropriate services. By developing and strengthening relationships with the residents, the Coordinator will be able to make referrals for such services as health care, nutrition, transportation and substance abuse prevention. In addition, the Coordinator will continue to provide assistance to LHA Management staff in handling issues with this population.

Boston Housing Authority --- $222,537

The Boston Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $222,537. Grant funds will be used to continue to utilize four(4) Service Coordinators. These funds will enable elderly and/or disabled residents to live independently, participate in and contribute to the community. The Boston Housing Authority collaborates with numerous community institutions and non-profit agencies.

Brockton Housing Authority --- $208,744

The Brockton Housing Authority located in Massachusetts will be funded for $208,744 for a One-year Service Coordinator Renewal Grant. Grant funds will be used to hire two (2) Service Coordinators at Caffrey and Manning Towers. These grant funds will continue to connect elderly/or disabled residents to needed and requested services, such as community service providers and other independent living, self-sufficiency educational, cultural enrichments and employment programs.

Worcester Housing Authority --- $232,404

The Worcester Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $232,404. The Housing Authority will provide assistance to seniors and non-elderly disabled residents living in housing authority apartments throughout the city of Worcester, MA. The services required by residents to live independently and avoid premature institutionalization will be determined. These Coordinators will arrange to match the needs of residents with services available in the community.

Lawrence Housing Authority --- $50,000

The Lawrence Housing Authority (LHA) received a $50,000 Service Coordinator Grant. The LHA will establish a meal-site at 353 Elm Street that addresses the concern of poor nutrition and creates a center for education and social interaction. The long term benefits of the program will bring about a ripple effect having a direct positive impact on the resident's physical and emotional health. The grant will enable the LHA to make a valuable contribution to the quality of life for our elderly-disabled.

Malden Housing Authority --- $50,000

The Malden Housing Authority received a $50,000 Service Coordinator Grant for a One-year renewal. The Malden Housing Authority seeks to continue and expand its comprehensive plan to address the varied needs of elderly and disabled residents. This Multi-faceted approach recognizes the need for familiar involvement and cultural sensitivity while coordinating the efforts of public agencies, private corporations, community organizations and aging professionals. This partnership is designed to enhance the quality of life for independent living and preclude premature, costly and unnecessary institutionalization. The Service Coordinator Program has enhanced the lives of many residents. The need, however, remains constant and substantial.
