Housing Commission of Anne Arundel Co. --- $31,611
The Housing Commission of Anne Arundel County has received a $31,611 Service Coordinator Grant. The Service Coordinator Grant will be used to fund one full time Human Service Caseworker at the Burnwood Gardens Public Housing Community in Glen Burnie, Maryland. Burnwood Gardens was the first public housing community constructed in Anne Arundel County, Maryland and houses both elderly and disabled families, many of whom require a myriad of essential health, emotional and psychological services.
Hagerstown Housing Authority --- $50,000
The Hagerstown Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $50,000. The Housing Authority will continue the services to help senior and disabled residents stay independent in their home. The Service Coordinator will provide intensive case management to individuals with more serious physical and mental health issues, assure that residents are linked to all available community services to which they entitled, and assist in bringing education programs to our residents on a variety of health related topics. The Service Coordinator Grant will also assist residents whose illnesses and handicaps are putting them at risk of loosing their housing due to health reasons by providing short term emergency services.
Housing Authority of the City of Cumberland Maryland --- $31,135
The Housing Authority of the City of Cumberland Maryland will be funded for a One-year Service Coordinator Renewal Grant in the amount of $31,135. The Housing Authority will provide services for the elderly and/or disabled. Our service coordinator currently provides knowledge of special needs of all the elderly and/or disabled residents in all five of the Authority's developments, which are Jane Frazier Village, John F. Kennedy Apartments, Benjamin Banneker Apartments, Fort Cumberland Homes, and Queen City Tower.
Housing Opportunities Commission of Montgomery Co. --- $52,378
The Housing Opportunities Commission of Montgomery County was awarded a $52,378 One-year Renewal Service Coordinator Grant. The Housing Authority will provide supportive services and activities to three elderly and disabled residents residing in public housing. The Service Coordinator will continue to provide services to the elderly and disabled with a focus on assessment, early intervention, crisis response and intervention and ongoing information, referral and linkage that will assist the residents to achieve the highest level of self-sufficiency possible. All elderly /disabled residents needing case management and referral services will be eligible to work with the Service Coordinator although the handicapped and disabled with greatest need will continue to be targeted.
