Port Huron Housing Commission --- $31,750

The Port Huron Housing Commission location in Michigan will be funded for $31,750 for a One-year Service Coordinator Renewal Grant. Grant funds will be used to hire one (1) Service Coordinator. These grant funds will continue to connect elderly and/or disabled residents to needed and requested services to live independently, participate in and contribute to the community and within their own resident housing developments.

Plymouth Housing Commission --- $32,640

The Plymouth Housing Commission jointly with the Ann Arbor Housing Commission has received a $32,640 Service Coordinator Grant. Plymouth Housing Commission serves as the lead agency, fiduciary, and administrators of the Service Coordinator program for both the Plymouth Housing Commission and the Ann Arbor Housing Commission. Plymouth Housing Commission has only elderly and their needs are handled on a "as needed" basis. Upon request, the Service Coordinator performs a needs assessment and assembles all available service agencies that can assist that senior. Ann Arbor Housing has mixed populations and they not only have individual "as needed" requirements of the agency, but have an ongoing program that attempts to meld the two populations.  

Flint Housing Commission --- $56,650

The Flint Housing Commission (FHC) received a $56,650 Service Coordinator Grant to extend its program for one year. The Housing Commission's Service Coordinators Program is designed to assist seniors in the areas of health, transportation, and support services. These services will be provided at the FHC Senior sites which include: Richert Manor, Forest Park, Garland, Mince Manor, Centerview, and Kenneth Simmons Square. The supportive Services for our Senior residents include: On-site nursing services, on-site pediatric care, book mobile services, family social services and caregiver services. The senior services program also places high priority on field trips and social events which the Housing Commission deems essential for aging adults.
