Minneapolis Public Housing Authority --- $229,500
The Minneapolis Public Housing Authority (MPHA) will be funded a for One-year Service Coordinator Renewal Grant in the amount of $229,500. The MPHA's FY 1999 Service Coordinator Grant is administered through a collaboration with Volunteers of America of Minnesota/Senior Resources (VOAOM/SR.) which historically has provided social services for elderly (62 + years of age) high-rise residents. The Service Coordinator Grant funds the salary of seven service coordinators who provide additional services for frail/elderly and non-elderly disabled residents. Services include: needs assessment, case management, referrals, monitoring of service utilization, linkages to mental health, job training, health and community service. Service coordinators work closely with MPHA staff, and other service vendors to ensure a continuity of services to the residents.
St. Paul Public Housing Agency --- $196,100
The St. Paul Public Housing Agency (PHA) will be funded for One-year Service Coordinator Renewal Grant in the amount of $196,100. The Service Coordinator grant will continue the salary and benefits costs for two Service Coordinators to refer, monitor and provide access to the services that public housing frail elderly and disabled residents require to live successfully independently. St. Paul PHA will also contract for coordination services in areas of lease compliance/independent living skills training and in bringing a vocational/educational/volunteer opportunities component on site in public housing hi-rises. The PHA will also contract for coordination services for a small number of residents needing to transfer to Section 8 housing options because their mental health issues preclude successful hi-rise living.
South St. Paul and Dakota County HRA --- $53,469
The South St. Paul Housing and Redevelopment Authority and Dakota County Housing and Redevelopment Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $53,469. The two Public Housing Authorities will continue to contract with Dakota Area Resources and Transportation for Seniors (DARTS) to provide Service Coordinators for residents of three buildings. Service Coordinators will be responsible for assuring that elderly residents, especially the frail or at risk, and non-elderly disabled residents are linked to supportive services needed to continue to live independently and to age in place. Service Coordinators will work with community providers to coordinate the provision of services that are tailored to each individual's needs. They will develop a system to monitor and evaluate the delivery, impact, effectiveness and outcomes of supportive services under this program.
