Housing Authority of Kansas City --- $54,346

The Housing Authority of Kansas City, Missouri received a $45,346 Service Coordinator Grant. The grant will provide service at 3 senior developments of the housing authority. The Authority also provides transportation assistance to seniors through the Tenant Associations of each of the Senior Developments (Pemberton Heights, Dunbar Gardens, and Brush Creek Towers).

Housing Authority of the City of Columbia --- $24,000

The Housing Authority of the City of Columbia will be funded for a One-year Service Coordinator Renewal Grant in the amount of $24,000. The Housing Authority will provide a coordinator for the Oak Tower. Oak Towers, is an eight story building housing elderly individuals and person with disabilities. The Coordinator will help residents obtain supportive services that are needed to enable independent living, and acts as a referral resource to connect residents with available local services, such as transportation, food recreation, clothing, and other necessities.

ST. Louis Housing Authority --- $39,461

The Saint Louis Housing Authority was awarded a $39,461 One-year Service Coordinator Renewal Grant. The Housing Authority will provide supportive services and activities to the elderly and disabled residents residing in public housing. The Service Coordinator for Elderly and disabled persons provides case management services which assist the client in the following ways: accessing resources, resolving and mediating conflicts with family members, identifying financial resources for the client. A case management plan is in place for each client and all cases receive regular monitoring and follow-up.
