Elmira Housing Authority --- $41,070

The Elmira Housing Authority will use its $41,070 Elderly Service Coordinator Grant for the services of one full-time coordinator. The Coordinator's time will be divided between the George E. Bragg Towers and Edward Flannery Apartments. The Coordinator will help residents obtain assistance to permit them to function independently. The Coordinator provides case management information/referral, advocacy/assistance, and counseling. The Coordinator will arrange for community agencies to provide any other specific services. The Housing Authority will contract with the Chemung County Office for Aging to facilitate the Service Coordinator. Grant money ($41,070) will be used for salary and fringes. Additional expenses will be provided by the Authority and the Office for Aging as in-kind contributions.

Rochester Housing Authority --- $184,375

The Rochester Housing Authority (RHA) will be funded for a One-Year Service Coordinator Renewal Grant in the amount of $184,375. The Housing Authority will continue its existing Service Coordinator Program which provides services that promote independent living at 11 public housing developments for the elderly. RHA's Service Coordinator Program currently provides our senior residents 24 hour access to the services of a service coordinator. The service coordinators will arrange for the required services necessary to promote independent living through a community agency, or in some cases, directly provide the service.

Syracuse Housing Authority --- $199,030

The Syracuse Housing Authority (SHA) will be funded for a One-year Service Coordinator Grant in the amount of $199,030. The Housing Authority will provide services for the residents. An initial assessment of each tenant is performed to see what services are needed. Service Coordinators work with tenants to ascertain what services should be put into place, and then follow up every three months for evaluation purposes and coordination of ongoing case management. Besides services for individuals, there are many group activities provided. The program increases the level of tenant safety and allows SHA to better serve its residents through case management, training and education, and crisis intervention. Improved access to on-site services may also reduce the need for unnecessary hospitalizations and moves into nursing or group homes; unit turnover is reduced and apartments remain rented.

Niagara Falls Housing Authority --- $76,500

The Niagara Falls Housing Authority was awarded $76,500 for a One-year Service Coordinator Renewal Grant. The Housing Authority will continue to provide supportive services and activities to elderly and disabled residents residing in public housing. These grant funds will enable the Housing Authority to continue to have two full-time supportive service coordinators.

Hampstead Public Housing Authority --- $58,000

The Hampstead Public Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $58,000. The Housing Authority will continue to provide service to the needs of frail and at risk elderly, as well as individuals with disabilities. The Housing Authority will also provide case management and referral services to senior and non-senior disabled residents; Build support and networking alliances with other community agencies; develop and maintain a Directory of Services; schedule and conduct meetings with residents on an as needed basis, both group and individual; meeting with local community and state agencies/organizations for input and program support.
