Housing Authority of Portland Oregon --- $182,400
The Housing Authority of Portland Oregon will be funded for a One-year Service Coordinator Grant in the amount of $182,400. The Housing Authority will provide Service Coordination to elderly and younger disabled residents living in public housing. The goal of the Service Coordinator Program is to assist these residents obtain the services they need in order to maintain housing stability. Specially trained staff link residents with needed services, supply information and referral, provide short-term intervention, and act as a liaison and advocate. The goal in providing these services is to preserve the resident's housing, while promoting independence, dignity and quality of life.
Housing Authority and Urban Renewal Agency of Polk --- $35,797
The Polk County Housing Authority and the Housing Authority of the City of Salem have joined in an endeavor to promote independence and avoid premature institutionalization of regional seniors and people with disabilities. They will be funded for a One-year Service Coordinator Renewal Grant in the amount of $35,796.75. Each Housing Authority will be able to continue to employ a Service Coordinator who will identify services that provide independence enhancing services, educate residents about those services. The Polk County - Salem Service Coordination effort will focus on linking public housing residents with in-place community services to promote independence and prevent premature institutionalization.
