Lancaster City Housing Authority --- $43,648
The Lancaster City Housing Authority (LCHA) will be funded for a One-year Service Coordinator Renewal Grant in the amount of $43,648. The Housing Authority will provide limited case management and referral services for close to 300 elderly and disabled public housing residents currently residing in Lancaster City Housing Authority's 2 hi-rises, Church Street Towers and Farnum Street East. A wide range of supportive services are provided by the Service Coordinator on-site. Some examples of those services include, assisting residents in completing entitlement forms, i.e., public assistance, food stamp, Social Security, and rent rebate forms, assisting residents to resolve conflicts with utility and credit companies, and to assist residents through personal crises which are often brought on by a death in the family.
Altoona Housing Authority --- $28,755
The Altoona Housing Authority received a $28,755 Service Coordinator Grant. The Altoona Housing Authority will partner with Blair Senior Services to provide a Elderly Service Coordinator for the residents of the Green Avenue and Eleventh Street Towers. The Coordinator will work with the residents to ensure that services are provided to assist the residents in maintaining a healthy independent lifestyle in their apartment community
Housing Authority of the County of Dauphin --- $50,000
The Housing Authority of the County of Dauphin's Elderly Service Coordinator Grant will be funded in the amount of $50,000 which will be used to provide on going technical services in order to facilitate the activities of the Senior Citizen Service Coordinator and to insure compliance with all applicable federal regulations. The senior citizen coordinator conducts the majority of interviews, assessments and follow-up visits in the home of residents. The service coordinator also attends tenant association meetings and conducts group training in the senior citizen apartment building's community rooms. Transportation for the residents is also available from the Dauphin County Transportation Service.
Bucks County Housing Authority --- $27,000
The Bucks County Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the Amount of $27,000. The Housing Authority will provide services to the elderly and disabled residents and linked their needs to supportive services. Through the Service Coordinator, residents experiencing problems which threaten their health, safety, and continued independent living are put in contact with those community service agencies which are best able to provide assistance to them.
Housing Authority of the County of Beaver --- $110,000
The Housing Authority of the County Of Beaver in Pennsylvania will be funded in the amount of $110,000 for a One-year Service Coordinator Renewal Grant. The grant funds will be used to hire three (3) Service Coordinators. This will continue to help elderly and/or disabled residents obtain supportive services that are needed to enable independent living and aging in place.
Housing Authority of the City of York --- $35,984
The Housing Authority of the City of York will be funded for a One-year Service Coordinator Renewal Grant in the amount of $35,984. The Housing Authority will provide continued funding for the employment and support of a service coordinator in York Housing Authority's public housing developments designated for the elderly and/or persons with disabilities. The service coordinator will assist residents to obtain supportive services that are needed to enable independent living and aging in place.
Housing Authority of the County of Butler --- $33,750
The Housing Authority of the County of Butler will be funded for a One-year Service Coordinator Grant in the amount of $ 33,750. The Housing Authority will continue to provide services to approximately 460 elderly and non-elderly disabled families, identify frail, elderly residents at-risk for premature or unnecessary institutionalization. Once identified, social services linkage with area agencies is implemented. The end-result enables residents to be better informed and familiar with the services needed, to age comfortably, and safely in place. The Service Coordinator will act in the information and referral capacity as well as liaison to social service agencies for the residents and their families. The Coordinator will strive to actively involve family members in the process of securing the appropriate care and services for their aging loved one.
Housing Authority of Northumberland County --- $30,000
The Housing Authority of Northumberland County received a $30,000 Service Coordinator Grant for one-year. The grant will be used to continue to educate residents on service availability, application procedures and client rights. At least two group education sessions are to be held at the Milton Towers to help residents to build support networks with other residents, family and friends. The Authority's Service Coordinator Program will continue to provide linkage with agencies and service providers in the community and monitor ongoing services of existing agencies.
Philadelphia Housing Authority --- $200,254
The Philadelphia Housing Authority (PHA) will be funded a One-year Service Coordinator Renewal Grant in the amount of $200,254. The Housing Authority will provide for case management and the coordination and linkage of services for the elderly and person with disabilities. The PHA's grant will provide direct services, including homemaker/chore services such as meal preparation, shopping an cleaning for the disabled only and exercise and fitness equipment for elderly residents. The Service Coordinator Grant will serve residents at twelve (12) senior and family developments. The PHA's grant will serve the disabled at all 50 PHA developments, including the ten (10) new scattered Sites Community Based Management Office Areas and the exercise and fitness equipment will be installed at seventeen (17) senior and family developments.
