Chattanooga Housing Authority --- $241,370

The Chattanooga Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $241,370. The Housing Authority will continue its partnership with Family and Children's Services of Chattanooga in providing special assistance to Housing Authority elderly and disabled. The grant allows for an array of services for the frail elderly and disabled so that they can continue to live in public housing instead of moving to a nursing facility. Services include assistance with grooming, eating and budgeting.

Knoxville's Community Development Corporation --- $306,870

The Knoxville Community Development Corporation will be funded for One-Year Service Coordinator Renewal Grant in the amount of $306,870. These funds will be used to provide case management services to elderly and persons with disabilities residing in 11 public housing locations in the city of Knoxville. Referrals will be accepted from all sources, including residents themselves. Follow up with local service providers will ensure that high quality service is delivered. The population served will be educated as to the obligations of tenancy, eligibility for local services and how to access them, entitlements, and clients' rights. Residents will be encouraged to participate in recreational and support group activities to promote independent living and enhance quality of life.  

Memphis Housing Authority --- $153,000

The Memphis Housing Authority (MHA) will be funded for a One-year Service Coordinator Renewal Grant in the amount of $153,000. The Housing Authority will provide supportive services to its elderly and non-elderly residents to allow long independent living and to prevent premature institutionalization. The Service Coordinator program will be operated at MHA's designated elderly and non-elderly disabled high-rise buildings.
