Housing Authority of the City of Dallas --- $53,485

The Housing Authority of the City of Dallas will be funded for a One-year Service Coordinator Renewal Grant in the amount of $53,486. The Housing Authority will provide a service coordinator to coordinate supportive and health services for residents at four public housing developments. The Service Coordinator will work with residents and managers at Park Manor as well as service agencies to develop wellness programs for the residents. The Coordinator also will assess the current service offered to these residents to maintain their ability to remain in their homes rather than be transferred to nursing homes or hospitals.  

Laredo Housing Authority --- $35,577

The Laredo Housing Authority (LHA) will be funded in the amount of $35,577. The Elderly Services Coordinator implements and coordinates the comprehensive delivery of services to elderly and disabled residents of the LHA. The LHA has two developments where 100% of residents are elderly, (Senior Citizens Home and Meadow Acres Elderly). The service Coordinator maintains an ongoing relationship with elderly and disabled service providers (such as home health agencies), schools, local government agencies, the resident councils and program volunteers. The Elderly and Disabled Services Program works closely with the resident councils, as activities are planned and delivered to clients.  

Fort Worth Housing Authority --- $59,623

The Forth Worth Housing Authority (FWHA) will be funded for a One-year Service Coordinator Grant in the amount of $59,623. FWHA is contracting with Mental Health Mental Retardation of Tarrant County for the Service Coordinator, who will continue to conduct comprehensive needs assessments of the elderly and disabled residents from Hunter Plaza and Fair Oaks Communities (approximately 297 residents), and perform individual case management. The Service Coordinator develops, coordinates and refers residents to community social services providers in order to meet their educational, physical, medical, social, and mental health needs. This promotes independent living and harmony among residents. The program is enhanced through contributions from the FWHA operating budget.

Temple Housing Authority --- $31,304

The Temple Housing Authority received a $31,304 Service Coordinator Grant. Temple Housing Authority is Successfully keeping frail elderly and person with disabilities out of nursing homes and is making it possible for these residents to live independently. This program has been in place since receipt of the 1995 Service Coordinator Grant. The Authority's Service Coordinator Program is administered by a licensed Social Work Associate of the State of Texas. Each of Temple Housing Authority's elderly and disabled residents are contacted annually and a subset of residents needing care are identified. These frail elderly/disabled receive more intense support and are contacted by the administrator on a more frequent basis.  

City of Waco Housing Authority --- $31,170

The Housing Authority of the City of Waco received a Service Coordinator Grant in the amount of $31,170. The Service Coordinator Grant will fund the services of a service coordinator to provide case management to our elderly residing in South Terrace and Estella Maxey. This coordinator will make referrals, provide counseling, education and activities at these sites to enhance the quality of life for our residents. This coordinator will also be part of a team within our community to advocate for public housing elderly and disabled issues within our community.

Housing Authority of the City of San Antonio --- $289,615

The Housing Authority of the City of San Antonio was awarded $289,615 One-year Service Coordinator Renewal Grant. The Housing Authority will provide supportive services and activities to 25 elderly developments and disabled residents residing in public housing. There are a total of nine Service Coordinators. The Service Coordinator's program complements two other programs for the elderly funded by the Office on Aging: Nutrition and Transportation.
