Housing Authority of the City of Seattle --- $240,975
The Seattle Housing Authority (SHA) was awarded a $240,975 One-year Service Coordinator Renewal Grant. The Housing Authority will provide supportive services and activities for the elderly and disabled residents residing in public housing. The SHA Service Coordination Plan incorporates the use of a community building in conjunction with comprehensive case management to provide a coordinated service delivery system. Four contracted service coordinators will provide outreach, engagement and case management services (including assessment, interim counseling, referral, and advocacy) to individuals who are at risk. Service Coordinators will also provide on-going consultation and training to SHA management staff to enhance their skills in identifying and referring clients to available case management programs.
Tacoma Housing Authority --- $47,925
The Tacoma Housing Authority was awarded $47,925 for a One-year Service Coordinator Renewal Grant. The Housing Authority will provide supportive services and activities to the elderly and disabled residents residing in public housing. The Service Coordinator will provide services at Tacoma Housing Authority's seven Senior apartment complexes.
Housing Authority County of King --- $252,687
The Housing Authority of the County of King will be funded for a One-year Service Coordinator Renewal Grant in the amount of $252,687. The Housing Authority will provide crucial assistance to low-income elderly and disabled residents living in 18 Public Housing developments in King County. The Service Coordinator Grant provides these frail and vulnerable residents with vital assistance, advocacy and coordination services to link them to services necessary to live independently within their homes. The grant helps these residents to avoid premature or unnecessary institutionalization and helps prevent the need for more expensive care, such as nursing home placements.
