The City of Tucson/Community Services Department will be funded for a One-year Service Coordinator Renewal Grant in the amount of $51,000. The City of Tucson will continue to promote independent living among elderly and non-elderly with disabilities the Service Coordinator will provide and/or refer targeted public housing residents those services, such as counseling, case management, crisis intervention, and community service access, as deemed needed by on-site professionals, whose long-term goals include minimizing unnecessary institutionalization's. Applicant proposes to retain local service provider (Family Counseling Agency) in a effort to build on trust and confidence established between said provider and the targeted residents over the past three (3) years.

California

Housing Authority of the City of Oakland $89,983

The Oakland Housing Authority will be funded for a One Year Service Coordinator Renewal Grant in the amount of $89,983. The Housing Authority will utilize the skills of a Service Coordinator to establish, maintain and nurture vital support service linkages for our elderly and disabled residents. We have found that many individuals need guidance through the changing landscape of complicated aging and disability issues, funding and agencies. Oakland Housing Authority senior and disabled residents rely on, and respond positively to the safety net of community referrals, information and services which the Service Coordinator makes available to them on-site. The Service Coordinator is a necessity in our agency's ongoing effort to identify and meet the many needs of residents.

Housing Authority of the County of Kern $45,144

The Housing Authority of the County of Kern (HACK) will be funded for One-year Service Coordinator renewal grant in the amount of $45,144. The HACK will provide comprehensive coordination of available support services to the elderly/disabled population of Kern County. The Service Coordinator has developed a provider network of stakeholders that offer important services that support autonomy and self-sufficiency among HACK's elder/disabled residents. Services provided to HACK's elderly/disabled residents. Services provided to HACK's elderly/disabled population through the coordinator's office included food baskets, health education, health screenings, and transportation services. The Service Coordinator provides case management services and guides clients through the process of obtaining and retaining public assistance and necessary supportive services.

San Diego Housing Commission $55,985

The San Diego Housing Commission will be funded for a One-year Service Coordinator Renewal Grant the amount of $55,985. The Housing Commission's Service Coordinator Program will provide employment and support services to elderly and/or persons with disabilities who are residents in four public housing developments. A total of 353 residents from four public housing developments listed below will be receiving services. The consistent quality of services available to the elderly in the Housing Commission's public housing sites has allowed the elderly to independently age in place. The service Coordinator Program will seek to continue to provide support services to allow elderly persons to live independently.

Connecticut

Housing Authority of the City of Norwalk $37,638

The Housing Authority of the City of Norwalk will be funded for a One year Service Coordinator Renewal Grant in the amount of $37,638. The Housing Authority will continue with the WeCARE Program (Weekly Coordinated Assistance in Residences for the Elderly). It is an "aging in place" program that is operated by the Senior Services Coordinating council under contract. The two part-time Service Coordinators provide referral services and direct services on-site at least weekly to residents of five senior public housing developments and one family public housing development in Norwalk. The services are provided at least 226 frail and at risk residents aged 62 or older, and disabled persons of any age.

Florida

Housing Authority of the City of Ft. Pierce $39,150

The Housing Authority of the City of Fort Pierce will receive a $39,150 One Year Service Coordinator Renewal Grant. The grant funds will continue the employment of a service coordinator throughout thirteen (13) public housing developments. The Housing Authority will provide supportive services and activities to the elderly and disabled residents residing in public housing.

Jacksonville Housing Authority $54,896

The Jacksonville Housing Authority will be funded for One-year Service Coordinator Renewal Grant in the amount of $54,896. The Housing Authority will hire one full-time and one part-time Services Coordinator to provide services to 615 elderly/disable residents at Centennial Towers, Hogan Creek and Twin Towers, public housing communities. The grant will create a collaboration with community agencies to provide additional supportive services such as housekeeping services, transportation, personal care, emergency response systems, and home delivered meals. The Service Coordinator will provide case management and referrals to residents, develop case plans, and increase networking opportunities for residents to improve their quality of life.

Georgia

The Housing Authority of the City of Augusta, GA $105,672

The Housing Authority of the City of Augusta, GA will be funded for a One-year Service Coordinator renewal grant in the amount of $105,672. The Housing Authority will employ two service coordinators to assist elderly and disabled public housing residents to obtain supportive services. These supportive services will enable the elderly and persons with disabilities to continue living independently and help them avoid premature institutionalization. The service coordinators will work with community service providers to coordinate the delivery of services.

Illinois

Housing Authority of Marion County, IL $30,000

The Housing Authority of Marion County located in Centralia, Illinois, will be funded for a One Year grant renewal in the amount of $30,000 to continue the employment of a Senior Resident Service Coordinator to provide and/or coordinate services for the elderly and disabled residents in Greenwood Acre I, Executive Apartments and Greenwood Acres III. These funds will enable elderly and/or disabled residents to live independently, participate in and contribute to the community.

Housing Authority of the City of East St. Louis $225,000

The Housing Authority of the City of East St. Louis will be funded for a One Year Service Coordinator Renewal Grant in the amount of $225,000. The ESLHA will continue its case management services, outreach, follow-up, education, referral and linkage to medical/social services for the elderly/disabled residents of the ESLHA.

Housing Authority of the County of Lake, IL $27,000

The Housing Authority of the County of Lake will be funded for a One year Service Coordinator Renewal Grant in the amount of $27,000. The Authority will contract with Catholic Charities of Lake County to provide case management and referral services for elderly and disabled resident at four of its senior citizen developments. Catholic Charities work closely with Illinois Department of Aging and administers a variety of programs designed to meet the needs of older residents in Lake County. These services will be extended to 198 households in which an estimated 35 are occupied by frail elderly and 15 are occupied by elderly persons believed to be at risk. The majority of residents of these buildings are single females with an average age of approximately 75 years. Thirty two percent of the targeted service population have annual incomes at or below the poverty level.

Rockford Housing Authority  $55,335

The Rockford Housing Authority (RHA) located in Chicago, Illinois will be funded for a One Year Service Coordinator Renewal in the amount of $55,335. RHA through a contract with a third party, community services provider agency, provides two Service Coordinators for two high rises Brewington Oaks, Park Terraces and three low rise developments. There funds will enable elderly and/or disabled residents to live independently, participate in and contribute to the community.

Indiana

Housing Authority of the City of Fort Wayne $39,056

The Housing Authority of the City of Fort Wayne will be funded for a One-year Service Coordinator Renewal Grant in the amount of $39,056. The Housing Authority will continue to provide elderly service coordination to all of our elderly and non-elderly disabled families residing in FWHA owned housing. We use the "How-to Guide for Service Coordination Elderly Person and People with Disabilities" as the program management plan. We currently contract with Aging and in Home Services because of their ability to provide high quality case management and liaison to other service providers for our elderly families. We provide weekly on-site availability of case management, education, and other health screening services to identify problems and build relationships with residents.

Iowa

Ottumwa Housing Authority  $31,980

The Ottumwa Housing Authority will receive funding for a one year Service Coordinator renewal grant in the amount of $31,980. The Housing Authority will provide supportive services and activities to the elderly and disabled residents residing in public housing through cooperative arrangements with Southern Iowa Mental Health Center, Seneca Area Agency on Aging, Retired Senior Volunteer Program, Ottumwa Regional Home Care, Wapello County Public Health Department, and Southern Iowa Economic Development Association.

Kentucky

Housing Authority of Henderson $33,519

The Housing Authority of Henderson will be funded for a One Year Service Coordinator Renewal Grant in the amount of $33,519. The goal of the Housing Authority of Henderson is to aid in meeting individual household, personal social, and recreational needs of elderly/disabled residents in order to prolong self-sufficiency. The elderly/disabled population of the Housing Authority represents 63% of this Housing Authority's total households or over 270 families. A qualified Service Coordinator, employed by the Housing Authority, will network with other community agencies in order to provide a variety of services including assessments, referrals, advocacy, case management, transportation, information assistance, activities scheduling, health promotions, and counseling.

Maryland

Housing Authority of Baltimore City $297,222

The Housing Authority of Baltimore City will be funded for a One-year Service Coordinator renewal grant in the amount of $297,222 (HABC). HABC presently manages 17 facilities that include 3,822 units devoted to the provision of housing for the elderly and/or disabled. HABC requests funding for the continuation of seven (7) existing Elderly Service Coordinator positions. The Service Coordinators assist HABC's elderly and/or disabled residents by providing linkages to supportive services that allow residents to maintain a status of independent living and to prevent institutionalization. The Coordinators responsibilities include the development of relationships with existing service providers, the facilitation of service delivery, and the monitoring/evaluation of the service units as well as any other duties that enable the residents to maintain their independence.

Massachusetts

Cambridge Housing Authority  $48,500

The Cambridge Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $48,500. The Housing Authority will provide service coordination to elderly and disabled residents at the John F. Kennedy Apartments. This 83 unit development has been selected by HUD to receive $5 million in HOPE VI funds for revitalization. As we plan for the redesign and upgrade of the building and the relocation of residents during construction, the role of the service coordinator will be critical. Ensuring the continuity of services for frail, at-risk elders during this particularly disruptive time will be essential to their well-being.

Michigan

Jackson Housing Commission $30,600

The Jackson Housing Commission will be funded for a One-year Service Coordinator Renewal Grant in the amount of $30,600. The Commission will continue and expand the efforts to meet the needs of residents at risk of placement in care facilities. With the support of existing community resources we will link residents to appropriate service providers that will help them maintain activities of daily living and keep individuals living in their own homes at our public housing developments.

Minnesota

Housing and Redevelopment Authority of Duluth, MN $43,985

The Housing and Redevelopment Authority of Duluth will receive funding for a one-year $43,985 Service Coordinator Renewal Grant. The Service Coordinator will continue to provide services to older adults an persons with disabilities who live in the public housing building sites designated in this grant. The services provided include: 1. Service Coordination and referral. 2. Problem solving, conflict resolution, and crisis assistance. 3. Advocacy for residents or linking to other advocates. 4. Education and training for residents and staff. We estimate that 250-275 persons per year will be served under this grant. Approximately 50 persons per year will receive direct service or referral elsewhere, to address a problem that impacts housing.

Mississippi

Housing Authority of the City of Biloxi $30,000

The Housing Authority of the City Biloxi, Mississippi will be funded for a One Year Service Coordinator Renewal Grant in the amount of $30,000. The renewal will allow the Housing Authority the opportunity to continue to ensure that elderly and non-elderly disabled residents continue to live independently, and participate in and contribute to the community.

Tennessee Valley Regional Housing Authority $50,000

The Tennessee Valley Regional Housing Authority (TVRHA) will be funded for a One-year Service Coordinator Renewal Grant in the amount of $50,000. The Housing Authority will continue to ensure that their 225 frail elderly, disabled or handicapped residents have access to the services that they need to live independently, to enhance their lives, and to prevent premature institutionalization. The service coordinator will provide case management, make referrals, and link residents to appropriate service providers in the community. These services will address the greatest needs of elderly and handicapped residents in TVRHA housing including, but not limited to health, income maintenance, transportation, and outlets for socialization.

New York

Albany Housing Authority  $64,787

The Albany Housing Authority will be funded for a One Year Service Coordinator Renewal Grant in the amount of $64,787. The Housing Authority will continue to funded two Service Coordinators for its Public Housing program in the City of Albany. They will provide case management and referral services to all elderly and disabled residents and their immediate families to ensure that all housing and related needs are met. Service Coordinators will work together and incorporate the available services from a large number of local agencies. The program will also serve to improve the marketability of our efficiency and one-bedroom apartments. Coordinators will include and AHA staff person and a third-party contracted service agency.

Buffalo Municipal Housing Authority  $157,968

The Buffalo Municipal Housing Authority (BMHA) will be funded for a One Year Service Coordinator Renewal Grant in the amount of $157,968. The BMHA will hire three (3) additional coordinators to assist the coordinators hired with the 1995 Service Coordinator grant to assist frail, elderly and disabled receive needed health and human services. The coordinator acts as the liaison between the resident and the Housing Authority as well as the resident to remain a tenant of the Housing Authority. It is estimated that this grant will service approximately 250 residents on an on-going basis.

Jamestown Housing Authority $50,276

The Jamestown/Dunkirk Housing Authorities will be funded for a One Year Service Coordinator Renewal Grant in the amount of $50,276. Service Coordinator Renewal Funding will be earmarked for a continuation of existing programs, specifically designed and implemented enrich the lives of public housing seniors and disabled adults. This collaborative project effort between the Jamestown and Dunkirk PHA's has proven extremely successful in terms of combating the negative impact posed by aging in place. In the case of both agencies, the funding received has allowed for the addition of a full-time Resident Advisor and a part-time Activities and Transportation Director. Additionally, ROSS monies have been instrumental in providing much improved access system to both preventative and acute health care services.

Plattsburgh Housing Authority  $35,640

The Plattsburgh Housing Authority (PHA) will be funded for a One Year Service Coordinator Renewal Grant in the amount of $35,640. The PHA's Service Coordinator works with the Senior and disabled residents (reviewing needs, offering advise, providing case management, and assists in obtaining discount programs) which increases their ability to remain the their homes. Twice, due to an established trust, a Senior's life was saved by obtaining needed medical care. The Senior Coordinator works with the Nutrition Program for the elderly addressing nutritional needs. This partnership results in healthy seniors that participate in more activities and live more productive independent lives.

Schenectady Municipal Housing Authority  $50,000

The Schenectady Municipal Housing Authority(SMHA) will be funded for a One Year Service Coordinator Renewal Grant in the amount of $50,000. The Housing Authority will continue their services unlimited program to provide assistance to senior citizens and disabled persons. Services Unlimited provides general case management, links with community services, assistance with completion of applications, promotions of activities and also works as an advocate for residents. The Services Unlimited office provides information and various presentations to residents, which in turn enables residents to make informed choices when facing major changes in their lives. The Services Unlimited office works with local mental health departments to establish group meetings and in the future is looking to establish a mental health office on-site for residents. The Services Unlimited office is vital to senior and disabled residents at SMHA. At the Services Unlimited office residents know they have someone who will both listen and help them - no matter the size of the problem.

North Carolina

Greensboro Housing Authority  $55,000

The Greensboro Housing Authority will be funded for a One-year Service Coordinator Renewal Grant in the amount of $55,000. The Housing Authority will use these funds to pay the salary of the Service Coordinator and cover administrative costs. The Service Coordinator will provide services to elderly and persons with disability residents, and obtain supportive services that are needed to enable independent living and aging in place.

Ohio

Jefferson Metropolitan Housing Authority  $30,000

The Jefferson Metropolitan Housing Authority (JMHA) will be funded $30,000 for a One-year Service Coordinator Renewal Grant. JMHA Service Coordinator Program links elderly, frail, at-risk, and/or disabled residents to supportive services and/or medical services in the community. This enables residents to continue independent living. The primary goal of the JMHA's Program is to continue networking with community service providers to meet the individual needs of each eligible resident. The purpose of this function is to promote self-sufficiency and independent living, while preventing unnecessary institutionalization for these residents. The second goal of the JMHA's Service Coordination Program is to evaluate all aspects of the delivery of these services. This allows for continuous quality improvement of all services that are provided.

Rhode Island

Cranston Housing Authority  $37,500

The Cranston Housing Authority will be funded for a One Year Service Coordinator Renewal Grant in the amount of $37,500. The Service Coordinator will assist the elderly and disabled population in six developments to live independently and participate in and contribute to the community.

Housing Authority of the City of Newport $30,000

The Newport Housing Authority will receive funding for a one-year Service Coordinator renewal grant in the amount of $30,000. The Service Coordinator will provide services for the elderly and person with disabilities for the Newport Housing Authority and also the Jamestown Housing Authority. Activities will take place at Donavan Manor. Various activities take place in these rooms such as bingo, arts and crafts, movie night, meals in a social setting and many health related workshops.

Housing Authority of the City of Woonsocket, RI $73,011

The Woonsocket Housing Authority will be funded for a one-year Service Coordinator Renewal Grant in the amount of $73,011. The Resident Service Coordinator working with Housing Authority of the City of Woonsocket, Rhode Island currently provides services to four Elderly/Disabled high-rises of the 749 residents these buildings nearly 10% are frail elderly and person with disabilities and another 8.5% are estimated to be at-risk. Renewal of funding for this position will assure continual delivery of services to these facilities.

Tennessee

Metropolitan Development and Housing Agency $96,364

The Metropolitan Development and Housing Agency (MDHA) located in Nashville, TN will be funded for One-year Service Coordinator renewal grant in the amount of $96,364. The MDHA will provide services to over 2000 elderly and disabled persons. The grant will provide salaries and fringe benefits for two staff persons which will expand the present services to this population of public housing residents. The Service Coordinators, through the provision of general case management and the linkage with needed supportive services, will ensure that elderly resident age in place with dignity and that the disabled residents live independently with a minimum of problems for management.

Texas

Housing Authority of the City of Austin $63,025

The Austin Housing Authority will be funded for a one-year Service Coordinator Renewal Grant in the amount of $63,025. The Service Coordinator grant will be used to employ two Service Coordinator who will initiate and support partnerships with local community agencies that provide on-site services to the Austin Housing Authority's elderly and disabled residents.

Housing Authority of the City of Corpus Christi $30,000

The Housing Authority of the City of Corpus Christi of Texas will be funded for a One Year Service Coordinator Renewal Grant in the amount of $30,000. The Service Coordinator will assist all elderly and disabled residents at Clairelaine Gardens, Parkway Homes and Ruthmary Price Place. These funds will enable elderly and/or disabled residents to live independently, participate in and contribute to the community.

Housing Authority of the City of Galveston, TX $38,697

The Galveston Housing Authority will be funded for a one-year Service Coordinator Renewal Grant in the amount of $38,697. The Authority will continue to assist the elderly in public housing in obtaining identified and needed services that they otherwise would not be able to access. This process will improve the quality of life of seniors by allowing them to age in place longer land to avoid premature institutionalization. Service coordination will be centered around the identification of needs, the linkage of providers to those in need, the development of relationships between the focus population and other community support agencies and advocacy on behalf of the focus population. Coordination will also create exposure for educational and interest experiences for the senior population that will enhance morale and continued independent living.

Virginia

Alexandria Redevelopment and Housing Authority $38,064

The Alexandria Redevelopment and Housing Authority (ARHA) will be funded for a One-year Service Coordinator Renewal Grant in the amount of $38,064. The ARHA will employ one senior services coordinator for 392 total units designated for elderly and disabled in public housing and section 8 housing. The focus of this program will be on 260 residents living on the premises at the Ladrey High-rise and Annie B. Rose Buildings. The Senior Services Program will offer an array of services and referrals involving medical, dental, mental health, legal, food, clothing, personal care, financial management, social, and spiritual needs for elderly and disabled residents of the Ladrey High-rise and Annie B. Rose House, and ARHA's other public housing units. The Senior Service Coordinator works in concert with resident managers, local government and private agencies, churches, civic groups, and other community entities to ensure timely delivery of services.

Marion Redevelopment Housing Authority  $225,000

The Marion Redevelopment Housing Authority (Lead Applicant) and nine Public Housing Authorities will be funded for a one-year service Coordinator Grant renewal in the amount of $225,000. Nine public housing authorities in Southwest Virginia have combined forces to provide their elderly, handicapped, and disabled public residents with significantly expanded and enhanced supportive services. This group of housing authorities has successfully administered a FY95 Service Coordinator Program, thereby avoiding premature and/or unnecessary institutionalization. As in the past, the targeted population's needs will be assessed, case management will be performed, and direct purchases will be made, purchases which include but are not limited to eyeglasses, dentures, and devices necessary for independent living. As our number of seniors citizens continues to grow, it becomes more imperative that their needs received increased attention.

Wisconsin

Appleton Housing Authority  $20,000

The Appleton Housing Authority will be funded for a one-year Service Coordinator Renewal Grant in the amount of $20,000. Oneida Wellness Program will keep residents living independently has been in existence since 1985. Through this program, the Coordinator encourages residents to use community services to they can maintain a functional level that will allow them to continue to live at Oneida Heights. Some community services that residents are referred to are: homemaking assistance, aging service, counseling, social services, meals on wheels, home care nursing aid, private nursing companies, and transportation. The entire wellness program promotes a continuity of care. The Service Coordinator acts also in the capacity of Nurse Advocate. Through these services, health surveillance, instruction, discussion, and support both physically and emotionally are offered to the residents. Through client-nurse interaction and referrals to physicians and community services, the residents maintain functional, physical, mental, emotional and spiritual health enabling them to continue living at Oneida Heights.
