Christian Care Manor IV proposes to convert 40 apartments to assisted living units. Renovations will be to bring 40 of the existing units to the maximum extent feasible into compliance with current applicable handicapped accessibility codes. The unit conversions from tubs to showers will accommodate the changing needs of the residents as they age in place, at least over the next ten years. Additionally, the owners plan to create new common space, staff offices, a commercial kitchen, dining room, commercial laundry facilities, and add an additional elevator to facilitate the delivery of services in an economic manner to the residents of the building.
Providing Assisted Living at Christian Care Manor IV, will allow frail elderly residents to receive the support they require, while remaining in an environment where they are comfortable.

Christian Care Manor II

Christian Care Manor II is proposing to renovate 24 units into assisted living units. In 2006, the owner received an ALCP Grant to convert 22 units to assisted living, along with the creation of new common space to include staff offices, a commercial kitchen, dining room, laundry facilities, and an additional elevator through other sources of funding. All renovations for this 2007 proposed project will bring the remaining 24 existing units into compliance with current applicable handicapped accessibility codes. The unit conversions from tubs to showers will accommodate the changing needs of the residents as they age in place over at least the next ten years.
Providing Assisted Living in Christian Care Manor II, a HUD Section 202 building, will allow frail elderly residents already living in an independent environment to receive the support they require, while remaining in an environment where they are comfortable. By providing Assisted Living in a HUD subsidized residence, residents are assured that decisions regarding housing are based on appropriate care needs rather than the ability to pay.

The Assisted Living Conversion Program grant funds will be used to convert 24 elderly units into assisted-living units for the frail elderly. Bonnie Brae is an existing 4 story building consisting of 164 units in 3 towers. The conversion will take place on the 3rd floor of the center building which is comprised of 23 studio apartments and one 1-bedroom unit. Renovations will provide handicap accessibility - widening doors to 36", wire type pulls on all kitchen cabinets and drawers, adjustable height closet shelving, higher wall outlets and lower wall switches and controls, a communication system between the new assisted living units and a new nurse's station, bathrooms will be completely renovated.
The owner is also creating a new nurse's station, an activity room, common kitchen with food warming equipment and refrigeration for the meal program, as well as a staff rest area for 24 hour oversight staff. Common area modifications will include corridor railings, a new accessible common therapeutic shower area and changing area. The existing laundry room will be made more accessible.
Providing Assisted Living in the Bonnie Brae Terrace housing development will allow frail elderly residents already living in an independent environment to receive the support they require, while remaining in an environment where they are comfortable.

The Assisted Living Conversion Program grant funds will be used to convert 10 existing independent units in the south wing of the elderly project. The ten units consist of four one-bedroom units and six-studio-units. All units will be redesigned and placed on a single corridor in this new scheme, allowing for added community spaces. This includes a living area, dining area, communal kitchen, laundry room, office/storage space, and monitoring stations. Renovation of the units will include a private kitchenette and fully accessible bathrooms.
The Assisted Living Conversion grant will allow for accommodation of the changing needs of the residents over the years. By renovating the wing in strict compliance, residents will be able to age in place, utilizing the special design features and meet their changing needs.
MINNESOTA

The Assisted Living Conversion Program grant will be used to convert 20 existing Section 202 elderly units to assisted-living units to accommodate aging in place residents at North Park Plaza. This conversion is a second phase to the previous conversion completed in 2005.
Renovations of the units will include kitchen and bathroom reconfiguration, (i.e., wheelchair clearances, wall reinforcement, grab bars, elevation of counters and work surfaces), bedroom, living and dining modifications, widen doors, upgrades to the common space, office, storage, monitoring stations, relocate emergency call devices, new electric load center, expand dining area, and add a vestibule between dining and exterior patio.
The Assisted Living Conversion grant will allow New Hope to achieve their mission to provide quality, affordable care to the elderly residents at North Park Plaza as well as continue providing multiple supportive services.
NEW JERSEY

The Assisted Living Conversion Program grant funds will be used to continue the conversion of elderly housing in this 229-unit, 14-story building to units that will serve the frail elderly. Phase II, involves the renovation of 17 existing elderly housing units on the 3rd Floor into 17 fully accessible units for the frail elderly. Unit modifications include the widening and overall modification of doors, as well as bathroom and kitchen renovations to comply with handicapped accessibility requirements.
Common area renovations on the first and third floors will also be accomplished. These modifications include corridor railings, electrical outlets, switches, lighting and flooring, as well as renovations to the Office area.
The unit conversions and common area renovations will facilitate the delivery of services in an economic manner and accommodate the future needs of the residents as they age in place.
NEW YORK
Syracuse
Bernardine Apartments, Phase VII

The Assisted Living Conversion Program grant funds will be used to continue the conversion of elderly housing in this 206-unit, 24-story building to units that will serve the frail elderly. Phase VII involves the renovation of 35 existing elderly housing units on Floors 12, 13, 14 and 15 into 30 fully accessible units for the frail elderly. Unit modifications include extensive upgrades to the bathrooms and kitchens, as well as closets and doors, to comply with handicapped accessibility requirements.
Common areas on Floors 12, 13, 14 and 15 will also be created, including two community lounges, a laundry and a dining area to serve the frail elderly residents. The unit conversions and common area renovations will facilitate the delivery of services in an economic manner and accommodate the future needs of the residents as they age in place.

The Assisted Living Conversion Program grant funds will be used to convert 32 units into assisted living units for the frail and elderly at Stygler Commons. The conversion will occur in four phases and minimize relocation and disruption in the lives of the existing residents.
The new addition will create space for a vestibule/lobby, new elevator, an office, congregate dining, full service kitchen, nurse's offices, and community space. Units will have new bathroom fixtures, walk-in showers, new HVAC systems, kitchen appliances, cabinets and countertops, and new flooring.
The 32 assisted living units are on a campus with two other senior sites and will allow a very much needed facility for aging in place residents to transition from independent living in an apartment to an assisted living facility.
